It seems spine is missing off the list? You can make a bionic spine and that's on the bionic pages, but missing here.

Part destruction on surgery fail

I noticed that nearly every body part page says that there is a chance that an installed part is destroyed if the operation fails, but to my knowledge it's not a chance, the part is always destroyed.

Am I wrong about that, or missing something?
